About PHP's Provider Network

Partners Health Plan (PHP) is proud to be the first managed care plan to be approved by the New York State Department of Health, Office of People with Developmental Disabilities (OPWDD) and CMS to exclusively serve persons with intellectual and other developmental disabilities (IDD) in New York City and four surrounding counties.

PHP’s Care Complete FIDA-IDD Plan provides the full continuum of Medicare- and Medicaid-covered services and supports, including OPWDD services, to adults age 21 or older who are dually eligible for both programs. This program is designed to fully integrate primary, specialty, and community-based services for some of New York’s most vulnerable residents.

Our ability to serve our participants effectively is dependent on the quality and accessibility of our provider network. You are one of the most critical components of our service delivery approach, and Partners Health Plan welcomes and appreciates your participation and dedication.

Partners Health Plan is a managed care plan that contracts with Medicare and the New York State Department of Health (Medicaid) to provide benefits to Participants through the Fully Integrated Duals Advantage for Individuals with Intellectual and Developmental Disabilities (FIDA-IDD) Demonstration.

The State of New York has created a Participant Ombudsman Program called the Independent Consumer Advocacy Network (ICAN) to provide participants free, confidential assistance on any services offered by Partners Health Plan. ICAN may be reached toll-free at 1-844-614-8800 (TTY users call 711, then follow the prompts to dial 844-614-8800) or online at icannys.org.
